Sarah Simonelli is a board-certified Psychiatric Mental Health Nurse Practitioner and founder of Evolve Psych Care. With over 14 years of experience in the mental health field—including 12 years in psychiatric case management and home-based care—Sarah brings a comprehensive and compassionate approach to care for patients of all ages. After earning her nurse practitioner certification, she trained under Dr. Hernandez and is also certified in interventional psychiatric treatments, expanding her ability to offer a range of evidence-based options. Her background includes leadership roles in healthcare management at Harbor Health and Cape Cod Healthcare.

Sarah raised her four children in both Western Massachusetts and on Cape Cod, where she now resides. In her free time, she enjoys running, cycling, gardening, cooking, and spending time with family and friends—especially by the ocean. She is passionate about helping others find emotional balance and well-being through personalized, patient-centered care that supports healing, resilience, and lasting growth.

Your first visit will be a comprehensive psychiatric evaluation lasting 45–60 minutes. We’ll discuss your history, current concerns, and treatment goals to determine the best care plan for you.

Not necessarily. Each treatment plan is individualized. Regular follow-ups will be scheduled to evaluate your progress and discuss options together, which may include tapering or discontinuing medication when appropriate.

Currently, appointment rates are set for cash pay, and health insurance is not accepted. However, you may be able to submit a claim to your insurance company for possible reimbursement.

No, Evolve Psych Care does not accept Medicare, as this is a private practice that accepts direct payment without health insurance.

At least 24 hours’ notice is required to cancel or reschedule. Late cancellations or no-shows will result in a $50 fee.

Like a traditional medical office, you will be asked to fill out online forms before your first appointment. This will include verification of your identification (driver’s license), medical history, and a list of medications you are on. For minors, a parent or guardian should plan to attend all appointments.
